In early 2026, Autumn Care of Nash ranked as the third largest nursing home in Nash County, data from the Centers for Medicare and Medicaid Services (CMS) shows.

Records indicate the nursing facility provided 60 beds, which was 46 fewer than Nash County’s average for the opening quarter of 2026. The home supported an average of 56.8 residents daily during this time.

CMS awarded Autumn Care of Nash an overall rating of 3 out of 5, above the statewide nursing home average of 2.9 for North Carolina.

Within the same quarter, The Carrolton of Nash ranked as the largest nursing home in Nash County with 141 beds available. Rocky Mount Rehabilitation Center was the second largest, offering 117 beds.

As of the first quarter of 2026, Autumn Care of Nash reported no fines or penalties.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
